  • Row 1 shows each field name
  • Row 2 shows examples of how each field should be formatted
  • Complete all fields except Claim Status
  • Use the correct Payer ID - check with us if unsure
  • Enter all dates in MM/DD/YYYY format
  • Enter Rendering Provider info only if different from Billing Provider
